Work Based Learning Training Agreement

The purpose of this document is secure permission for the above named to participate in The Cookie Cart Training Program.

Student’s Parent/Guardian

1. Gives permission for their child to take part in Cookie Cart's Employment Training Program.
2. Will provide support to their child concerning attendance, initiative, safety, and other favorable work traits.
3. Agrees to communicate with Cookie Cart staff as needed throughout the experience.
